Sometimes it just ends up finding enough of a mouth skin hook hold there whilst it is being sampled/picked up/ejected.
I Wouldn’t worry too much, your location is spot on & they want to eat your bait, that’s the important bit, you are landing tench as well.

Caught a 30lb grassy the other day which was hooked in the top lip. The hookhold was bomber, but I am wondering why and how in the top lip?

Solid bags, close range, holes in the weed. 4" hooklength, size 5/6 hook, 2.5oz lead.

Simple rigs, with wafters (that barely waft) and just a shrink tubing kicker.

I thought it was a one-off, but i also got tenched out and hooked one of them in the top lip too?

Any ideas?
