As a rule of thumb, I'd start by using about 1/3 of the levels you are suggesting and work from there. 3ml flavour, 1ml sweetener, depending on suggested inclusion rates from manufacturer.

When I roll corkballs I always go by the manufacturers recommended levels for a 1lb mix,
Like 2ml Flavour & 1ml Talin. Use that for a 1 egg mix.
Then take it from there. Less is more, you don't want them too strong.
With such as thin skin and you lose some through the boiling/drying process. You can always carefully rehydrate them bit by bit afterwards once dried in a flavour/talin/hydroslate type solution.

A lightbulb moment for me was Jason Hs podcast a while ago when he talks about the hookbait being the one thing you actually want then to eat - not try via attraction. I have never used a synthetic flavour that is palatable at high doses.
In his book Gerry hammond said he liked a high dosage flavour in his pop up mix.

Doesn't it all depend on the flavour, it's base and it's concentration? And one companies 5000-1 can be different to anothers depending on if it's cut before they get it, and what it's cut with.
10ml of the original Chocolate Malt to an egg would be find, 10ml of Tutti probably not.

This, its entirely dependant on the flavours. I know that Frank Warwick (and others) used to use Solars S&O at very high levels (like 20ml/ egg) in the past...

10ml S&O with 1ml of Esterblend 12 is minging, can still smell it in the shed two weeks after airdrying a batch.

I think you need to consider if you are going to fishing them as singles or over bait.
As someone has mentioned Frank Warrick has spoken about using extremely high levels of flavour, but this was predominantly for fishing as singles at range in large low stocked waters.
I think what the OP has posted sounds good. And also worth having a look at the sticky corkball rolling guide if its your first time
