Has anyone ever been checked on after filling in one of these?

On my second visit to see my brother in Canada customs did telephone to see if I was indeed staying there.

Just to update.....

I'm currently halfway through my 10 day quarantine and can confirm that I have been contacted every day so far. They keep you on the phone a good 3 or 4 minutes at least to make sure that you understand your obligations and to trace your call to make sure you are where you're supposed to be.

Calls are made from what seems like a downstairs toilet and are invariably made by employees whose accents are extremely difficult to understand. (Make of that what you will.)

Choose your test provider very carefully as mine (Dante Labs) have been shocking so far with items missing from kits (twice!) late results and absolutely no customer support.

As far as the form is concerned, it needs to be filled in no more than 48 hrs before departure and can only be completed when you have booked your 2 and 8 day tests back in the UK as you will have to provide laboratory reference codes.

Please don't think that you can get away without following any of these procedures either as you will be constantly checked.
